An off-site follow-up survey conducted on February 27, 2023, found that H & M Care, Inc., had corrected the following deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of an initial Medicare certification and licensure survey conducted on December 27, 2022 through December 29, 2022.
